For registration you have to fill in several forms. Before our appointment you will receive a list of the subjects that you have to decide on in the lving will. That will allow you to think about your wishes in advance and talk with friends or family to form an opinion on the choices to make.

Examples of choices in the medical will:

  • Appointing a representative
  • Your standards and values about quality of life
  • Medical procedures such as blood transfusion, feeding tube and sedation
  • Donation of organs and tissues
  • Spiritual assistance

During our personal appointment we fill in the forms and of course you can ask me any questions you have.

We take the completed forms with us to the appointment at the register. There the questions are reviewed again and entered into the computer. You will receive proof of registration in the register, as well as a signed and stamped copy of your living will.
