Once the forms have been completed, they must be officially registered with the Servicio de Salud of the Junta de Andalucía. During that appointment, your medical will will also be directly registered in the National Register of the Ministry of Health. Your living will is then immediately valid. You can view and change it at any time.
Registration offices
The registration of the living will is done by a specially designated official of the Junta de Andalucía. There are offices in Vélez-Màlaga, the Centro de Salud Las Lagunas in Mijas, the Hospital de Ronda, the hospital de Antequera and in Málaga in the Hospital Civil, Hospital Virgen de la Victoria and the Delegación Territorial de Salud.
The waiting time for an appointment is between four and six weeks.
Documentation
To make an appointment, a Número de Historia Única de Salud en Andalucía (NUHSA) is required. If you do not have this, I will request a number for you, that number will only be used to register the living will.

The appointment
During the appointment, the entire form is reviewed again and the data is entered into the computer. You can still ask questions. Unless you indicate otherwise, I will be present at this meeting to translate.
Afterwards you will receive a copy of your living will and proof of registration.
The appointment lasts half an hour to an hour.
Why do you have to come yourself
You register your personal wishes about what you want and don't want when you are dying. The government would like to be sure that you are the one who registers these wishes. That is why you must always come personally to an office when registering and also when making changes.
